Unhappy Extension (FHIR 3.4.0) not loading properly, Mac 10.11.5, Mirth 3.4.0

Hello - I am new to Mirth, and have successfully set up a local version to convert HL7 v2 to XML. This was just to get a feel for it.

I am now trying to install the FHIR extension, but have never installed an extension before. I am following the instructions, but for whatever reason the extension is not showing up as installed. Some things I tried:

1. Cleared the cache (all 3 options: Trace and Log files, Cached Applications and Applets, Installed Applications and Applets), exited the server and the Mirth Connect Administrator
2. Checked the file permissions, removed com.apple.quarantine attributes
3a. Install the extension from the file system (using the zip file)
3b. Copying the unzipped FHIR directory into the "Applications/Mirth Connect/extensions" directory

A few questions:
1. How can I debug this? The log file in the "Mirth Connect" directory is not showing anything abnormal... is there a way to access logs? Am I looking in the wrong space?
2. Are there good tutorials to install extensions?
2. I am not a premium user of Mirth Connect, and will not be - Are there any other open source extensions? Where can I find them? (I want to try and install other ones)
3. What could be happening?

Are there any errors? I tested on 3.4 and it appears to work fine for me (also on OSX).
Step 1: JAVA CACHE...DID YOU CLEAR ...wait, ding dong the witch is dead?

Always include what Mirth Connect version you're working with. Also include (if applicable) the code you're using and full stacktraces for errors (use CODE tags). Posting your entire channel is helpful as well; make sure to scrub any PHI/passwords first.
