Default Here is an example of markup needed for a Meaningful Use CCD

Here is Continuity of Care Example . We use Mirth and other interface engines to provide Hl7 feeds that are consumed by our clinical data repository to generate this document.

Post A question on suziecd.xml

One thing I noticed is that the Family name and Given name tags seem to be reversed? In other words, the Family name is Steve, while the Given name is Jones in the example.

Is this because the Family name always has to be first or des that matter?

Would you have a CCD example that contains an attachment?

I would like to have different types of attachment -jpg, pdf, docx, etc.

Thanks so much!

BTW: my real goal is to use a tool that can create my own test CCD's and CCD's with attachments. Any guidance much appreciated!

Hi ,
Can a CCD document contain free-form texts or it is a completely structured document.
For eg. can we find a discharge summary or physician notes inside the document?
